I hereby agree to the disclosure of all evidence concerning racial discrimination in pay, promotions, job assignments, discipline and terminations, and with respect to the creation of a hostile work environment, that I may have provided to Kodak or any of its consultants during my employment by Kodak, or that may have been provided about me. I hereby acknowledge and authorize that such information may be provided to counsel for the Plaintiffs in the race discrimination class action case captioned Employees Committed for Justice, et al. v. Eastman Kodak Company, Civil Action No. 6:04-cv-06098-JWF (W.D.N.Y.), and that such information may be used to support the claims made in this case, including class certification. I hereby waive any objection to the production of such information on all grounds, including that such information may have been initially provided to Kodak or its consultants on a confidential basis. I believe that I am a class member in this case, and fully support this case and the elimination of racial discrimination at Kodak.
